BetterRX

I hereby irrevocably grant to BetterRX, LLC (“BetterRX”), its affiliates and subsidiaries, assigns, agents and licensees, a non-exclusive, royalty-free, fully paid-up, transferable, sublicensable, irrevocable, perpetual, worldwide license to edit, reproduce, display, exhibit, publish, broadcast, distribute, and otherwise use, and permit others to use, my performance, image, voice, or physical likeness, in any form or media, as well as my personal information, including my name and biographical information (individually and collectively, my “Likeness”), in whole or in part, in connection with BetterRX’s marketing, advertising, promotion, production, distribution, and other exploitation of BetterRX’s products and services (the “Purpose”) .  

I agree that all rights in the video(s), recording(s), picture(s) or other materials capturing my Likeness in connection with the Purpose (the “Materials”), including all copyrights therein, shall be the exclusive property of BetterRX upon creation and I hereby relinquish, give and assign to BetterRX all right, title, and interest in and to the Materials including, without limitation, all negatives, reproductions, derivative works or other copy that may be used in connection with the Materials. I also hereby waive any right to inspect or approve the Materials that may be used by BetterRX.

I waive any claim to any compensation of any kind through the sale, license, transfer or any use of my Likeness and the Materials and, further, release and agree to hold BetterRX harmless from and against any claim or cause of action, including but not limited to defamation, invasion of right of privacy or publicity, or based upon or relating to the use and exploitation of my Likeness and the Materials.​

I warrant that I am over the age of 18 years, that I have read this Media Release in its entirety, and that I am voluntarily agreeing to the terms herein.
