Terminal Agitation in Hospice: Recognizing and Managing Restlessness at the End of Life
Managing terminal agitation in hospice goes beyond reaching for a sedative. It's recognizing the change early, assessing what's actually driving the restlessness, and responding in a stepwise way rather than automatically escalating medication. Comfort depends on two things equally: treating the agitation as a signal of an underlying cause rather than the diagnosis itself, and matching the response (environmental changes, medication, or both) to what's really happening with the patient. This article explains what terminal agitation looks like, how hospice teams assess likely contributors like pain, constipation, or medication effects, when medication such as haloperidol or a benzodiazepine becomes appropriate, and how to support families through a frightening but often manageable part of the dying process.
