Challenges with pharmacy-hospice direct contracting

Pharmacies face significant challenges when contracting directly with hospices.  Some of these include:   

1. Creating and managing contracts and contract terms. 

 

2. Chasing down and trying to collect funds owed by hospices (and losing money when the hospice doesn’t pay).  

 

3. Pharmacies can face conflict when a nurse insists on dispensing a medication that is not on the formulary. If the pharmacy complies, they risk being reprimanded by the Administrator and incurring the cost of the drug.
On the other hand, if they refuse to dispense, they risk being criticized for neglecting the patient’s needs. 

 

4. Taking orders via phone calls is very time-consuming.  

 

5. Client support and consultations require significant resources. 

 

6. Marketing and sales efforts are expensive.
